Passport Services

To make a new passport appointment at Huron Public Library, please call (419) 433-5009.
Before your new passport appointment, gather the following:
- Complete Form DS-11 using black ink only. (available at travel.state.gov or at the Huron Public Library.)
- Get an acceptable passport photo taken. Do not smile or wear glasses (Discount Drug Mart, CVS, Rite Aid, etc. offer these services)
- Bring a certified birth certificate or previous passport with you to your appointment.
- Bring a current driver’s license. (If you don’t have a driver’s license, bring a state ID, school ID, and your social security card.)
- Bring a check or money order made out to the US Department of State for a passport book or card. (If you want to fill out the amount later, that is okay. Expedited passports cost more.)
- Bring cash or a check for the Huron Public Library. (Presently $35.00 per passport)
If all of these items are not complete and present at time of appointment, Huron Public Library has the right to cancel/reschedule the appointment until all the materials are ready and available.
Huron Public Library does not make same-day appointments or allow walk-ins. All appointments are made on the basis of staff availability.
Passports for minors under 16 years of age
- Both parents need to be present unless you have a death certificate available or only 1 parent is listed on the birth certificate.
- If both parents cannot be present, visit travel.state.gov to see how to apply for a child’s passport.
Passport Renewals
- Most passports can be renewed without a passport agent’s assistance.
- Passports issued for children under the age of 16 and adult passports expired for more than 5 years require an agent’s assistance to complete.

Notary Services
Notary services are available by appointment only. To confirm availability of the notary and to make an appointment with the notary, call the Library’s main phone number at 419-433-5009.
Notary Witnesses
A witness must personally know the person and must be in possession of valid photo identification. The library will not provide witnesses, and library patrons cannot be solicited to be witnesses.
Required Identification
A customer seeking notary services must provide valid photo identification. Valid forms of photo identification are:
- Current driver’s license
- State-issued ID
- US passport
- US military ID
Documents
The following documents will not be notarized:
- Mortgage, housing refinance, and other real estate documents. Signing documents for real estate documents require additional training. Our notaries are not trained as signing agents.
- Last Wills and Testaments. In the state of Ohio, the last Will and Testament requires two witnesses. Notarized affidavits to create a self-proving Will are not valid in Ohio.
- Notary protests
- Homeland Security I-9 forms. I-9 signatures are not a notarial act in the state of Ohio.
- Foreign documents written in any language other than English when the notarial certificate is in a language other than English.
- Automobile, boat, or mobile home titles without seller or buyer name and address (state law requires this section to be complete regardless of circumstances); buyer may be insurance, donation, or salvage company with direct instructions for the signer on how to fill out the title. Notaries are unable to explain the title to the signer.
- Any document where the notary is asked to notarize their own signature.
- Medallion notary services (banks only).
- Financial power of attorney documents.
- Car title that is unusual or has already been signed by a seller who is not present, and/or a title with erasures, cross outs, and/or white out, open titles
No electronic or online notarizations are available.
Vital Records & Documents
The State of Ohio does not allow notaries to certify vital records and documents. Vital records include, but are not limited to passports, marriage certificates, and birth certificates. Instead, notaries can notarize an affidavit stating the photocopy is a copy of the original document.
What does this mean? We cannot certify documents. We can only notarize signatures.
What should I do? If you choose to have an affidavit notarized, bring the original documents to the appointment with the notary for inspection by the notary. Do not sign the affidavit prior to the appointment with the notary.
Other Policies
The notary and the customer must be able to clearly communicate directly with each other. Notaries are not permitted to make use of a translator to communicate with the customer.
Notaries cannot pre-date or post-date any action, prepare a legal document, give advice on legal matters, or notarize documents in which they have a personal interest.
