Passport Services at Huron Public Library

To make a new passport appointment at Huron Public Library, please call (419) 433-5009.

Before your new passport appointment, gather the following;

  1. Complete Form DS-11 using black ink only. (available at travel.state.gov or at the Huron Public Library.)
  2. Get an acceptable passport photo taken. Do not smile or wear glasses (Discount Drug Mart, CVS, Rite Aid, etc. offer these services)
  3. Bring a certified birth certificate or previous passport with you to your appointment.
  4. Bring a current driver’s license. (If you don’t have a driver’s license, bring a state ID, school ID, and your social security card.)
  5. Bring a check or money order made out to the US Department of State for a passport book or card. (If you want to fill out the amount later, that is okay. Expedited passports cost more.)
  6. Bring cash or a check for the Huron Public Library. (Presently $35.00 per passport)

If all of these items are not complete and present at time of appointment, Huron Public Library has the right to cancel/reschedule the appointment until all the materials are ready and available.

Huron Public Library does not make same-day appointments or allow walk-ins. All appointments are made on the basis of staff availability.

Passports for minors under 16 years of age

  • Both parents need to be present unless you have a death certificate available or only 1 parent is listed on the birth certificate.
  • If both parents cannot be present, visit travel.state.gov to see how to apply for a child’s passport.

Passport Renewals

  • Most passports can be renewed without a passport agent’s assistance.
  • Passports issued for children under the age of 16 and adult passports expired for more than 5 years require an agent’s assistance to complete.
